Amenity Access Request

Step 1: Sign the waiver and forms
Complete the Amenity Waiver and Access Request Form by clicking the purple button below. Make sure to read through the entire website page for important rules and information.
 
Step 2: Verification Process
Our team will review your information to confirm residency at Evergreen. Make sure you upload any required documents when filling out the form. Only one document is needed per household. This can be closing documents, utility bills, a lease etc. 
 
Step 3: Payment Info
Complimentary Access: Every household gets 2 free mobile accesses for one year. After that, each mobile pass is $30 a year.
Extra or Replacement Costs:
Physical Key FOB: $30 flat fee
Lost key FOB: $50 flat fee
Extra mobile access (per year): $30
If any payment is needed, we’ll send you a secure payment link via Square before distributing access.
 
Step 4: System Setup
Once everything is approved, we’ll add your information to our system and set up your access. For mobile access, you will receive an email from customercare@brivo.com with instructions on attaching your mobile credentials to the app. The Brivo link expires 7 days after it is sent. There will be a delay between submitting the forms and receiving the Brivo email and your FOBS key, as this is done manually. 
To use the Brivo mobile credentials at the Arbor, you must be connected to the amenity center's Wi-Fi.

Step 5: Picking up Key FOBS (If needed)
If you requested any physical key FOBs, you can schedule a time with clevantino@ccmcnet.com to pick them up at the Association's office at The Arbor during business hours, or request a drop-off at your front door. 
 
                                                                                     
 
 
Amenity Mobile Credential Process
Once you have completed the waiver, the Association Team will manually process residents into the Mobile Credential App, Brivo. Make sure to download Brivo on your mobile device and click the link provided in the email to transfer the mobile credential to the app. Over the next few days, please keep an eye out for an email from customercare@brivo.com. Be sure to check your spam folder! This link expires 7 days after it is sent.
 
Please note: This is a complete reset of all access systems, and the Association team will need to verify all addresses and homeowners, and input each individual into the Mobile Amenity Credential App. There will be a delay from submitting the forms to receiving the Brivo email. Please verify the accuracy of the email address provided, as this will be the channel through which the Mobile Amenity Access will be sent to you.
 
To use the mobile credentials at the Arbor, you must be connected to the amenity center's Wi-Fi.
